Pelvic Pain: CT vs Ultrasound — What You Should Know

Life Medical Imaging Erina Radiology Xray Ultrasound CT Scans

Pelvic pain is a common reason for medical imaging referrals. It can arise from a wide range of causes, including gynaecological, gastrointestinal, urinary, or musculoskeletal conditions. Imaging tests are often a key part of the diagnostic process, helping your doctor understand what might be contributing to your symptoms so that appropriate care can be provided.

At Life Medical Imaging, we offer a range of imaging services — including ultrasound and CT scans which can help your doctor investigate pelvic pain. Understanding the differences between these tests, and when one might be used over the other, can help you feel more informed and prepared for your appointment.

Understanding Ultrasound and CT

Before we look at when each test is used, here’s a brief overview of what they are:

Ultrasound

Ultrasound uses sound waves to create images of internal structures. It is widely used to look at soft tissues and organs in the pelvis, including the uterus, ovaries, bladder, and bowel. No radiation is involved, and the test is usually quick and comfortable.

CT (Computed Tomography)

CT uses X-rays to create detailed cross-sectional images of the body. It shows both soft tissues and bony structures. CT scans provide more detail than traditional X-rays, especially in complex cases, but they do involve a small amount of radiation.

When Is Ultrasound Usually Used for Pelvic Pain?

Ultrasound is often the first imaging choice for many cases of pelvic pain because:

  • It can assess gynaecological structures, such as the uterus and ovaries
  • It helps evaluate bladder and some bowel conditions
  • It’s useful for pelvic fluid collections and evaluation of masses
  • It’s safe (no radiation) and suitable for a wide range of patients
  • It can be used for obstetric imaging during pregnancy

Your doctor may recommend an ultrasound when your symptoms suggest issues such as:

  • Menstrual cycle-related pain
  • Suspected ovarian cysts
  • Fibroids
  • Uterine abnormalities
  • Pain during pregnancy
  • Bladder or urinary symptoms

Ultrasound is available at Life Medical Imaging locations on the Central Coast, and our team can answer any questions you have about preparing for the test or what to expect on the day. Learn more about our ultrasound services here.

When CT May Be Recommended

A pelvic CT scan provides a more comprehensive look at the pelvis and surrounding structures. It may be recommended when:

  • Your doctor needs a broad overview of both soft tissue and bone anatomy
  • Ultrasound findings are inconclusive
  • Symptoms are acute or severe (for example, suspected appendicitis or kidney stones)
  • Your doctor needs to assess complex anatomy or deep-seated structures
  • There is suspicion of conditions affecting organs beyond the reach of ultrasound

CT scans are particularly useful in cases of emergency or when other investigations have not provided enough information to guide care. They can help assess conditions such as:

  • Suspected appendicitis
  • Bowel pathology
  • Urinary tract stones
  • Complex trauma
  • Post-surgical assessment

Pelvic CT is performed at Life Medical Imaging’s Bateau Bay, Erina, and Umina clinic, where we use advanced CT technology to provide detailed imaging while focusing on patient comfort and safety.

Your doctor will consider your symptoms, medical history, and clinical findings when determining which test is most appropriate.

What You Can Expect on the Day

Ultrasound

  • A sonographer applies gel and uses a handheld transducer over your lower abdomen
  • The test is typically painless
  • It usually takes 15–30 minutes

CT Scan

  • You lie on a table as the scanner rotates around you
  • Some CT studies may involve a contrast injection for better detail
  • The test usually takes 5–15 minutes

Both tests are performed by experienced imaging staff who aim to make your visit as comfortable as possible.

Preparing for Your Test

Preparation can vary depending on the test ordered. Your referral or the clinic team will provide specific instructions, such as:

  • Fasting requirements for some CT protocols
  • A comfortably full bladder for certain ultrasound studies
